• Do you have new or changed medications?

• Do you have new or unhealing wounds?

• Have you recently been diagnosed with a disease?

Medical conditions that previously required hospitalization are now often treatable at home with home health care. Three criteria must be met for Medicare eligibility: ​ 1. The patient is currently a Medicare recipient 2. The patient is homebound 3. The patient's physician must order skilled care as medically necessary

Medicare's definition of homebound is described as:

• Requires assertive devices

• Confusion

• Unable to leave home unassisted

• Shortness of breath

• Needs assistance for all activities

• Requires assistance to walk or move about

Yes. All home health care occurs under the order and supervision of your primary care physician. We will work with you and your physician to determine a plan of care that meets your needs.
